Terms & Conditions

Conditions of purchased packages:

1. This care plan expires after _________ months from the date of purchase.
2. A patient may choose to cancel their care plans at any time while it is still valid, and a refund will be given for the entire unused portion of said plan. An administrative charge of $125.00 will be charged for all cancelled packages.
3. All care plans are per individual and cannot be shared between family members.
